The Air Accidents Investigation Branch (AAIB) has published an AAIB Special Bulletin S2/2012 on the incident involving EC225 LP Super Puma, G-REDW, 20 nm east of Aberdeen on 10 May 2012.
The helicopter carried out a controlled ditching after indications of a failure of the main gearbox lubrication system and, subsequently, a warning indicating failure of the emergency lubrication system.
Passengers and crew evacuated in a life raft and were rescued, the passengers having sustained minor injuries.
The investigation found a 360° circumferential crack in the bevel gear vertical shaft in the main gearbox - in the vicinity of a manufacturing weld - which caused disengagement of the drive to both mechanical oil pumps.
